T1 General

Specialists & Surgeons: Our Analysis

Specialists are the clearest example of mixed supply in Canadian healthcare. Medically necessary procedures are exempt, while purely cosmetic procedures are generally taxable, and many surgical practices do both. Once you have taxable billings you must apportion input tax credits between the exempt and taxable sides rather than claiming everything or nothing. Third-party reports and medical-legal work are the other commonly missed taxable stream.

What a Accountant Checks First in Specialists & Surgeons

Before we quote a fee or open a file, we ask specialists & surgeons owners the same first question: who prepared this last, and did they work the sector regularly? The answer usually predicts what we will find.

Ask any accountant where specialists & surgeons files go sideways, and the answer usually traces back to this: Shareholder loan balances must be repaid within one year of the corporation’s following year-end or the amount is included in the shareholder’s personal income under subsection 15(2).

There is a companion rule that changes how the first one plays out in practice: Accrued but unbilled revenue belongs in income in the year the work was performed. Deferring it to the invoice date understates taxable income and is a standard reassessment adjustment.

If any of this maps onto your situation, the next step is deliberately low-stakes: we quote a fixed fee up front, prepare the work, and you review everything before you pay. Sector files are what we do all week — bring us yours.

Specialists & Surgeons: the tax rules that actually apply

Every engagement we run for Specialists & Surgeons starts from the same question: which healthcare rules apply to this file, and which of them is the client currently getting wrong? These are the ones that come up most.

What the CRA looks at

Locum arrangements are contracts for service, and paying a locum without a written agreement or a business number invites a worker-classification review.

A clinic mixing exempt treatment with taxable supplies (retail products, cosmetic procedures, medico-legal reports) must apportion its input tax credits, and the CRA reviews that split closely.

What you can actually claim

Leasehold improvements to a clinic are Class 13, amortised over the lease term rather than expensed, which surprises practices that have just renovated.

Clinical equipment generally falls in Class 8 at 20%, while computers and diagnostic software sit in Class 50 at 55% — the classification decides how fast the cost comes back.

The filing calendar that applies

Practitioners who bill provincial health insurance receive payment statements that must reconcile to reported revenue — mismatches are the single most common CRA query in this sector.

Where the planning value sits

Incorporation only pays once the practitioner leaves meaningful profit in the company; where the whole income is drawn, the corporate structure often costs more than it saves.

Because passive income above $50,000 grinds the small business deduction, a practice retaining profit needs its investment portfolio structured with that threshold in mind.

None of this is charged as extra advice. It is simply what a properly prepared file for Specialists & Surgeons includes, at the fixed fee agreed up front.

Straight Answers on Specialists & Surgeons Filing

Direct answers to what Canadian business owners actually ask before hiring an accountant.

What instalments does a specialists & surgeons business have to pay?

Individuals pay quarterly instalments once net tax owing passes $3,000 in the current year and either of the two preceding years. Corporations generally pay monthly, with many small CCPCs eligible for quarterly instalments instead. Basing them on a current-year estimate avoids overpaying after a strong year.

What tax deductions are specialists & surgeons businesses most likely to miss?

The recurring ones are capital cost allowance on equipment placed in service late in the year, the business-use portion of vehicle and home-office costs, and professional development. Each is defensible when documented at the time and difficult to defend when reconstructed later.

How much does accounting for specialists & surgeons businesses cost?

Corporate tax filing starts at $90 and bookkeeping at $10 per month, quoted as a fixed fee before work begins. Sector complexity does not trigger a surcharge. Review the full price list.

Do specialists & surgeons businesses need to register for GST/HST?

Registration is mandatory once taxable revenue exceeds $30,000 over four consecutive quarters. Registering voluntarily below that threshold is often worthwhile, because it lets you recover input tax credits on startup and equipment purchases.

Should I incorporate my specialists & surgeons business?

Incorporation usually pays off once profit consistently exceeds what you draw personally, because retained earnings are taxed at the small business rate rather than your marginal rate. Below that point the added compliance cost often outweighs the benefit. We model both before you decide.

What records do specialists & surgeons businesses need to keep?

The CRA requires six years of books and records from the end of the tax year they relate to: invoices, receipts, bank statements, payroll records and contracts. Digital copies are acceptable provided they are legible and complete.

How do you handle payroll for specialists & surgeons businesses?

We run the cycle, remit source deductions on schedule, and issue T4s ahead of the February deadline. Late remittances draw a penalty of up to 10% and repeat lateness raises it to 20%, so timing is the whole game. See our payroll service.

Can you work with my existing bookkeeping software?

Yes. We work in QuickBooks, Xero, Wave, Sage, spreadsheets, and plain scanned documents. You are not required to migrate systems to become a client, and we never charge a conversion fee.

What does "Pay After Service" mean?

Our Pay After Service model means you review and approve all deliverables before making any payment. We prepare your returns or financial files, you review them, and only then do you pay. This ensures 100% satisfaction.

How does price matching work?

If you find a lower verified quote from another Accounting Firm in Canada for the same scope of services, we will match it immediately. Simply provide a verified quote.

How do I submit my tax documents?

We support completely secure digital uploads via our client portal, or you can email them to us. We support files from QuickBooks, Xero, Excel, and scan/photo documents.

Do businesses in specialists & surgeons need a different kind of bookkeeping setup?

There is a widespread assumption here, and the actual position is worth stating plainly. Capital cost allowance is permissive, not mandatory. A corporation can claim less than the maximum in a low-income year and leave the undepreciated capital cost in the pool for a year when the deduction is worth more, provided the schedule carries that decision forward consistently. If your current setup was built on the assumption instead of the rule, that is fixable — but sooner is better than later.

What tax issues come up most often in the specialists & surgeons sector?

Related-party transactions have to be recorded at fair market value, and a below-market charge between connected companies invites an adjustment on both sides of the transaction. We flag this early with every client it touches, because finding it out at filing time leaves you far fewer options than finding it out now.

More Specialists & Surgeons Questions Canadians Ask

The questions Canadians actually search on this topic, answered plainly. Browse every question in the Canadian tax answers directory.

Most municipalities do not take credit cards for property tax directly. They accept pre-authorised debit, online or telephone banking, cheque, and in-person payment. Third-party payment processors will charge a property tax bill to a card for a service fee, which normally costs more than the rewards earned. The CRA works the same way for income tax and GST/HST: no direct card payment, but authorised third-party providers accept cards for a fee.

Yes. Rent you receive is taxable income. You report the gross rents, subtract deductible expenses such as mortgage interest, property tax, insurance, utilities you pay, repairs, condominium fees and advertising, and the net rental profit is added to your other income and taxed at your marginal rate. There is no separate rental tax rate. A rental loss can generally offset other income where the property is genuinely rented at fair market value.

Property tax is an annual charge a municipality levies on real estate to fund local services such as roads, water, policing, fire and schools. A provincial assessment authority values the property, council sets a tax rate in its budget, and the bill is the assessed value multiplied by the rate for your property class. Unpaid amounts stay attached to the property itself. It is separate from income tax, so the city bills it, not the CRA.

Zoning matters because assessment and taxation follow a property's classification and actual use. Residential, multi-residential, commercial, industrial and farm classes carry different municipal rates, so rezoning or a genuine change of use can move a property into a class taxed differently. Assessed value can also reflect development potential permitted by the zoning. Ask your municipality and your provincial assessment authority what a rezoning would do to the class and the bill before you apply.

There is no single figure, because the amount depends on your income, your province and which taxes are counted. Studies that quote an average usually bundle income tax, payroll contributions, sales tax and property tax together, which is why published numbers differ so widely. For your own position, divide the total tax shown on your notice of assessment by your total income to get your effective rate, then compare that with later years rather than with a national average.

A T4E is the slip Service Canada issues for Employment Insurance and certain related benefits. It shows the total benefits paid, income tax already withheld, any benefits you had to repay and any benefit repayment required because of your income level. Report the amounts on your personal return on the line for Employment Insurance and other benefits, not on the employment income line — EI benefits are taxable but they are not employment income. Keep the slip even where no tax was withheld, because the benefits remain taxable and the CRA already holds a copy.
